The Community Alignment Specialist (focused on prenatal to age three) is a member of Ready for School, Ready for Life (Ready Ready), a backbone organization focused on building an innovative, connected early childhood system in Guilford County, North Carolina. The Ready Ready team convenes key partners and galvanizes the community to: 1) increase the number of children born healthy; 2) ensure that more infants, toddlers and preschoolers are on track developmentally; and 3) increase the number of children who enter kindergarten ready for school and life success.
Guilford County has built a vision for assessing what families want and need, connecting them with the right services at the right time, using data to drive improved service provision, and positively impacting the lives of young children and their families. The Community Alignment Specialist will play a central role in connecting families to community resources based on identified family needs.
The Community Alignment Specialist will join a team focused on aligning community resources and services to meet the needs of families from pregnancy to age three. The Specialist will be part of a team focused on aligning people, processes, and supporting technologies within Guilford County to drive better outcomes for our youngest children and their families.

The work includes:
-Identifying agencies that serve families from pregnancy to age three, establishing a relationship with each agency, learning about its services and eligibility criteria, and codifying this information to enhance the system of care for families.
-Building, maintaining, and improving an Agency Finder tool (a comprehensive resource directory) that is critical for programs to connect families to needed resources at the time they need them.
-Working with the GRGI Navigation System to connect families with community resources that they need. This will include:
>Participating in staffing with Community Navigators to understand the needs of families, share community resources that could meet those needs, and support utilization and improvement of the Agency Finder;
>Providing information and data to the GRGI Navigation leadership team to guide quality improvement, align community resources, and assist with reporting.
-Troubleshooting with public and private partner agencies, organizations, and programs to help improve the referral process and the alignment of local resources.
-Representing the Get Ready Guilford Initiative with local coalitions.
-Supporting the Get Ready Guilford Initiative Navigation process to ensure all families are assessed for need at key points in time, connected with targeted interventions when needed, and provided ongoing support so no child slips through the cracks.
-Soliciting feedback from Get Ready Guilford Initiative Navigation partners about unmet community needs and, with Ready Ready staff, planning toward closing those gaps, as possible.
-Conducting outreach to support and market the Get Ready Guilford Initiative Navigation process.
-Establishing and maintaining communication between community resources, physicians, nursing personnel, and other health care professionals regarding Ready Ready’s objectives.
-Bachelor’s degree plus five years of progressive experience in the human services field and/or a master’s degree in a related field.
-Skill in interacting with human service providers and agency leaders
-Ability to communicate clearly and professionally with a range of community stakeholders.
-Leadership skills and a willingness to take initiative and be proactive.
-Ability to work independently, as well as part of a high-performing team.
